そもそもゴムを使えと指導し、さらに無料でゴムを配った上でさらにプラセボを用いたランダム化二重盲検試験を行っています。
よく勘違いされるのですが、ランダム化比較試験の対照群は、完全なプラセボではなく、その時点での標準治療でなければなりません。
HIV感染の予防は「コンドームを使え」というのが標準でしたから、さらにそれにHIV薬を加えることで追加の効果が期待できるか、というのが今回の試験で明らかにしたいことです。
抗HIV薬にも副作用があり、HIVに感染しなかったが腎不全になったとか、気持ち悪くなるので結局のめなかったとか、そういうことがあれば予防効果は実際にはありません。コンドームにさらに抗HIV薬を追加して効果があるかどうかは、「コンドーム(+偽薬)」という現状の標準治療と比較することによって初めて明らかになります。もちろん、これは現状の最良の予防策をとっているわけで、そうでなければ倫理的に臨床試験の実施は許されません。
もちろん、有意な結果が出た以上、偽薬に割りつけられた群にも今後は実薬を提供される機会が与えられなければなりません。抗HIV薬の値段を考えると、どのくらいの意味をもつのか分かりませんが。
